THIE Announces Patient Safety Grant Recipients
As part of its ongoing commitment to improving patient safety and promoting risk management, the Texas Hospital Insurance Exchange has awarded three grants of $5,000 each to subscriber hospitals for projects to improve upon or implement new patient safety policies and/or procedures targeted at reducing hospital readmissions occurring within 30 days of discharge. See the recipients below.

| Parmer Medical Center in Friona received a $5,000 grant to implement a new patient discharge education program designed to improve disease management and reduce readmissions by educating patients about their medical conditions and how to manage them. |

| Rolling Plains Memorial Hospital in Sweetwater received a $5,000 grant to implement medication management/reconciliation software that will help the hospital reduce readmissions by improving the process for medication reconciliation at the time of patient discharge. |
